Important Information on Office Policies and Insurance Plans
As a courtesy to our patients in Livingston, Summit, and throughout northern New Jersey, we will submit your claims for *medically necessary services* to your insurance carrier for reimbursement. All co-pays, co-insurance and deductibles that are part of your policy will be due at the time of service. In order to submit your claim correctly, we ask that you provide us with accurate demographic information, insurance information and a copy of a valid insurance card. If you do not see your listed carrier below, please contact us, as this list constantly changes.
Even if we are not in-network with your insurance, we may be able to help facilitate your care. We do not want financial issues to prevent you from obtaining medically necessary care.
Please keep in mind, your medical insurance policy is an agreement between you, the policy holder, and your insurance carrier. Your policy is one method to help you cover fees for services you use at Dermatology Consultants of Short Hills, but is not a substitute for payment. To avoid unexpected out-of-pocket expenses, we suggest learning as many details about your plan as possible, including co-payment amount, co-insurance amount, deductibles, referral requirements, covered services and exclusions from your policy.
Cosmetic services such as Botox, dermal fillers, laser hair removal, other laser treatments, and treatments for varicose veins are not covered by any insurance policy under any circumstances, and payment is due at the time of service. If you would prefer assistance in financing your procedure, we offer lines of credit through CareCredit.
